Who we are

What this page collects

• The name, phone number and email you type into the consult request form.
• Which ad or link brought you here (for example the campaign and ad IDs in the link), and how far you got in the form.
• A random visitor ID saved in your browser so a later form submission can be matched to the same visit.

This page does not ask for health information. Please don't include any; the team will go over everything at your consult.

How it's used

• To contact you about your consult request by phone, text or email.
• To measure which ads bring in consult requests.

Who it's shared with

• Accuvision's marketing partner, Iris Ascension, receives your request and passes it to the Accuvision team.
• To measure ads, we may send Meta (Facebook and Instagram) a hashed (scrambled) version of your email and phone with a generic "Lead" or "Schedule" event. We never send Meta the procedure you asked about or any health information.
• We don't sell your information.

To stop calls or texts, reply STOP to any text or tell the team when they call. To ask us to delete your request, call (704) 765-2745.
